The integration of telehealth into mental health and substance misuse treatment is not just a convenience; it's a pivotal shift in healthcare delivery that addresses some of the most pressing barriers to care. Data consistently show that telehealth increases access for individuals living in rural or underserved communities where mental health services are scarce. By leveraging technology, patients have the flexibility to engage in sessions without the constraints of geography, which is crucial given that nearly 111 million Americans live in mental health professional shortage areas. Furthermore, studies have documented that telehealth services can be as effective as in-person care, particularly for psychiatric conditions such as depression and anxiety, which often co-occur with substance misuse. This modality also enhances patient engagement and retention in treatment programs, vital for long-term recovery outcomes. Telehealth also offers a level of anonymity that can reduce the stigma associated with seeking help for mental health and substance use disorders. This is crucial because stigma is a significant barrier that prevents many individuals from accessing care. Unfortunately, the approach has failed to attain widespread adoption despite a wealth of operational advice from private and public sources and thoughtful and thorough policy recommendations to overcome implementation barriers. According to a rare quantitative estimate, approximately 20 percent of #primarycare physicians are co-located with 19 percent of #behavioralhealth clinicians (in fewer than 40 percent of primary care practices), which means that even smaller percentages have adopted behavioral health integration—more than 30 years after CoCM was developed and more than 20 years after randomized controlled trials and several meta-analyses affirmed its value. In every field of medicine, we follow a certain process, a traditional medical model. What is called Gender Affirming Care or the Affirmation model, however, is not a medical model. I have compared and contrasted the differences to provide health professionals, journalists, and politicians with a clear understanding of the landscape. See image below. I am happy with the work; it has been checked by four psychiatrists. My psychologist friend Nancy Wake and I are responsible for this work. Given the threat to employment that health professionals face for not being politically compliant, she is using a pseudonym. Why does this matter? Headspace has been lobbying doctors to provide puberty blockers within the first two visits. These are significantly risky drugs, with a considerable number of irreversible side effects.
