We are thrilled to share a recent case study highlighting the impact of Joy on a customer in Ellesmere Port.💪🚀 The One Ellesmere Port PCN Social Prescribing team initially faced challenges with capturing data, accessing insights, and allowing practice staff, in addition to GPs, to refer with over 70,000 patients across 6 GP surgeries. Joy became the solution, offering a comprehensive platform with data collection, caseload management, and integration with clinical systems to send and code referrals. 👨🏽🤝👨🏼📈🙌 The PCN Executive Administrator, Sam Minshall says “Joy has helped us hit our targets and exceed them by 4x. This gave us even more confidence that Joy is worth the investment”. The #research conducted by the University of Minnesota's Center For Practice Transformation highlights that individuals living in recovery housing while in intensive outpatient (IOP) produces positive outcomes in several areas compared to those that do not live in recovery housing during IOP. "CPT found that compared to self-housed people (e.g., living at a permanent address alone or with others, unhoused), those who lived in a recovery residence during their IOP treatment increased an average of 19 more days of sobriety from intake to 16 months post discharge, and were 3.8 times more likely to have an increase in sober days, 1.3 times more likely to have an improvement in depression symptoms, and 1.3 times more likely to successfully discharge from the IOP."
