See if you can focus on some of the key techniques such as clarify and verify or the creation of a strong problem statement, the next time you're on a case. Look for opportunities to gather data in an organized fashion using the 5W2H methods. Remember, perfect practice makes perfect. So be mindful of your technique and improve your confidence levels by attempting them regularly. Next, improve your knowledge with other process improvement tools and frameworks. My favorites include Six Sigma, Lean and Kepner-Tregoe. Root cause without execution will not lead to a reduction in incidents.
