PENGARUH KONSELING GIZI TERHADAP STATUS GIZI IBU HAMIL KEK PADA PROGRAM JPS-BK DI KOTA PALEMBANG

Background : Low quality and quantity of food and limited knowledge on nutrition are major cause of malnutrition among pregnant mothers. Nutrition counseling is expected to improve nutrition status of pregnant mothers.
Objective: To identify the effect of nutrition counseling to nutrition status of chronic energy malnourished pregnant mothers.
Methods : The study was a quasi experiment which used pretest – posttest control group design. Subject of the study were pregnant mothers with upper arm circle < 23.5 cm. The study was conducted at 2 health centers which had high prevalence of chronic energy malnourished pregnant mothers in Palembang Municipality. Nutrition counseling was given every 2 weeks within 3 months. Data obtained were food consumption every 2 weeks at 6 times (multiple recall), weight gain and measurement of upper arm circle once a month and knowledge of the subject about nutrition.
Results : There was difference in knowledge of mothers who got nutrition counseling from those who did not (p<0.05). There was difference in energy consumption of mothers who got nutrition counseling from those who did not (p<0.05). There was no difference in protein consumption between mothers who got nutrition counseling and those who did not (p<0.05). There was difference in weight and upper arm circle of mothers who got nutrition counseling and those who did not (p<0.05).
Conclusion : Nutrition counseling affected increase of knowledge, energy consumption, weight and upper arm circle. Nutrition counseling did not affect increase of protein consumption.

ANALISIS ZAT GIZI DAN BIAYA SISA MAKANAN PADA PASIEN DENGAN MAKANAN BIASA

Background : The quality of food service in a hospital can be assessed from the inpatients’ nutritional status. Food waste is an indicator of food service among inpatients. Besides its therapeutic value, food has a significant economic value. The wasting cost in term of food waste affects the total availability of food costs.
Objective : This study analyzes the nutrient quantity and the cost of food waste among inpatients with regular diet at Dr. Sardjito Hospital, Yogyakarta.
Methods : This was a cross sectional study. The subjects were inpatients aged 17 to 60 years old who got regular diet with length of stay was at least three days, and were willing to take part in this study (n=100). The amount of food waste was measured using the Comstock visual estimation. The cost of food waste was calculated as the proportion of food waste from cost per serving. The quantity of nutrients in food waste was calculated using the Food Processor 2 software. The data were analyzed using Chi-square test.
Result :There was a difference of food waste according to gender. Rice waste was found more frequent among female (p<0,005). There was a difference of food waste according to ward class. There were more waste of meat and vegetables among inpatients in class II and the difference was significant (p<0,05). There were more waste of meat and vegetables among patients with length stay of 7 – 14 days and > 15 days (p<0,05). The vegetables and rice waste were more frequent among surgery and cancer inpatients (p<0,05). In average the nutritional value of food waste was 19,85% - 9,33% of a patient’s RDA, while the wasting cost per day was Rp 1265,08 or 10,79% of all food cost per day. The annual wasting cost of food waste was Rp 45.543.120 or 4,4% of the available budget of Rp 1.038.605.333,00.
Conclusion : There were differences of food waste according to gender, ward class, length of stay, and kind of disease, especially rice, meat, and vegetables.

PENGARUH SUPLEMENTASI ZINK (Zn) TERHADAP DIARE PADA PENDERITA UMUR 6-36 BULAN YANG DIRAWAT DI RSUD Dr. SOEDARSO PONTIANAK, KALIMANTAN BARAT

Background : Diarrhoea is one of the major causes of infants and childhood death in Indonesia. Malnutrition and zinc deficiency in diarrhoea may lead to impaired immunity.
Objective : To assess the influence of zinc supplementation on the duration of diarrhoea and the frequency of watery stools in under-three-year-old children
Method : This was a double-blind randomized controlled clinical trial, with 31 children in the supplemented group and 33 children in the placebo group. All children had acute watery diarrhea and were given supplementation within 24 hours of admission. Daily supplementation of 20 mg zinc was given to the experimental group.
Result : The supplemented group had a shorter hospitalization compared to the placebo. Beginning from the second day of supplementation, the frequency of watery stools in the supplemented group was significantly less than in the placebo group (p<0.001). While there was a significant decrease in the concentration of zinc in the serum of the placebo group, there was a slight increase in the supplemented group.
Conclusion : Zinc supplementation can shorten hospitalization and reduce the frequency of watery stools in children with diarrhoea.

HUBUNGAN ANTARA ASUPAN ENERGI, ASUPAN LEMAK, DAN OBESITAS PADA REMAJA SLTP DI KOTA YOGYAKARTA DAN DI KABUPATEN BANTUL

Background : The prevalence of obesity in young children and adolescents has been increasing very dramatically during the last two decades, especially in big cities. High intake of energy and fat may have contributed to obesity.
Objective : To examine the relationship between energy intake, fat intake and obesity among adolescents.
Methods : A Case Control Study was conducted in 2003. Subjects were Junior High School students, age 10–15 years, living in the city of Jogjakarta and Bantul Regency were included in the study. Data on nutrition intake were collected by using Food Frequency Questionnaires from 140 obese and 140 non obese junior high school students.
Result : On average, the energy intake among cases and controls from the city of Jogjakarta were 2818,32 ± 618,05 kcal/day and 2210,42 ± 329,81 kcal/day respectively. Whereas total average of energy intake among cases and controls from Bantul were 2416,99 ± 673,52 kcal/day and 1778,09 ± 312,03 kcal/day respectively. Obesity was significantly associated (p < 0.05) with total energy and fat intakes in both city Jogjakarta and Bantul.
Conclusion : Obesity was associated with total energy and fat intakes in Junior High School Students.

HUBUNGAN CITRA TUBUH DENGAN STATUS OBESITAS, AKTIVITAS FISIK DAN ASUPAN ENERGI REMAJA SLTP DI KOTA YOGYAKARTA DAN KABUPATEN BANTUL

Background : The prevalence of obesity in adolescence, both in industrial and in developing countries, is increasing. This might be due to the fact that globalization has affected the lifestyle and the eating pattern of the adolescents.. Apart from related to higher risk of cardiovascular morbidity and mortality, obesity in adolescents might be also related to dissatisfaction of body image.
Objective : To asses the association between obesity and body image, and between body image and energy intake and physical activities of junior high school adolescents in the District of Yogyakarta and Bantul..
Method : This is a cross-sectional study. Subjects were 96 pairs of 12-15 years old obese and non-obese adolescents, identified in the obesity screening programs in junior high schools in Yogyakarta and Bantul. Subjects were asked to complete 34 questions on body shape image. Energy intake and physical activity data were obtained by food- and physical-activity-frequency questionnaires.
Result : Obese adolescents had higher odds to be dissatisfied to their body images compared to their non-obese peers, i.e. Mantel-Haenszel Odds Ratio (95% CI) of 14.6(6.2-34.4). Obese female adolescents had higher odds to be dissatisfied than their male counterparts, OR (95% CI) of 19.5(2.4-15.9) in female and OR (95%CI) of 13.4(5.3 – 33.8) in male adolescents. There were no significant associations between dissatisfaction status with energy intake and time spent on light physical activities. Dissatisfied adolescents tended to spend less time on hard physical activities compared to their satisfied peers.
Conclusion : Obese adolescents were more likely to be dissatisfied with their body images compared to their non-obese peers, but the dissatisfaction did not lead to less energy intake and more time spent on harder physical activities.

PENILAIAN GIZI SEDERHANA: ALAT PENILAIAN PRAKTIS UNTUK MENENTUKAN STATUS GIZI PASIEN RAWAT INAP DI RUMAH SAKIT

Background : Absence of effective and efficient screening tool for assessment of nutritional status have lead to high incidence of malnutrition among hospitalized patients.
Objective : To develope practical screening tool for assessing nutritional status of hospitalized patients.
Method : A simple method of nutritional assessment, called The Simple Nutritional Assessment, was compared to albumin serum as the gold standard. This study involved 50 nurses and 50 hospitalized patients in Dr. Moewardi General Hospital, Surakarta.
Result : There was significant concordance of nutritional status assessed by the Simple Nutritional Assessment and the level of albumin serum (k=0,92). Sensitivity, specificity, and likelihood ratio of the Simple Nutritional Assessment compared to serum albumin were 92%, 67–75%, and 3.8, respectively. Internal consistence of the Simple Nutritional Assessment is high (Alpha=0.80). Average time required for doing the assessment was only 5 minutes.
Conclusion : The Simple Nutritional Assessment can be used for quick screening of nutritional status of hospitalized patients with high sensitivity and reliability.
